Select the scream layer, apply Image > Adjustments > Level or Control+L to adjust the layer level. Adjust until you get this.

Open the image. Select Edit > Define Brush Preset…Name the brush 7. Now this will act as your brush now. Close the ink-splatter image. Now you should be in the project file. Select the BABY layer. Create a new layer over it. Select your foreground color as #cc0000. And click once over the baby image. You will get a red spot.
